This cancellation form needs to be submitted before 12 noon on the last day of the month in order to not be charged dues for the next month.

No refunds will be given for month charged.

All Perkville points are void at date of cancellation and can not be used after. Points are not transferable to other members. The freeze option retains rewards points.
